With a trixbox or any asterix system, for that matter, you could in  
theroy use any sip or iax provider.

I used voicepulse before in a production enviorment and at home.

The beauty with a homegrown voip box is that you are not tied to a  
single carrier. You can pre purchase blocks of mintues from different  
voip hosts. You can then create outbound rules for calls to certain  
area codes or countries.

With voip the posibilities are pretty open. Mostly depends on how much  
time you want to invest.
